The Consortium For Older Adult Wellness
 Helping Adults Manage Chronic Conditions

Learning how to live with chronic health conditions is a need shared by most adults and a skill that becomes increasingly important as we get older.  Success in healthy aging results in improved health, lower health care costs, and reduced demand on the health care system.

Learning to self-manage their chronic health conditions is important as adults grow older
In July 2009, SECAHEC expanded its activities conducted in southeastern Colorado through a partnership with the Consortium for Older Adult Wellness (COAW).  COAW is a 501(c)(3) organization dedicated to increasing the knowledge, attitudes, and practices of a diverse mix of professionals and policy-

makers who work on behalf of aging audiences in Colorado.   COAW has established a state-wide system which recruits and trains individuals from throughout Colorado to conduct courses for adults in their communities.  Among the course offerings is

Healthier Living Colorado™ which is a chronic disease self-management course that is evidence-based and was developed and researched by Stanford University.  The class is taught in English, Spanish, Vietnamese, Hmong, Laotian, Chinese, Japanese, and Filipino in Colorado.  Attendees include adults who have chronic diseases and conditions such as heart disease, arthritis, diabetes, asthma, bronchitis, emphysema or other chronic conditions.  They are encouraged to bring a family member or support person with them.  Other courses offered through COAW address healthy living and lifestyle skills such as Therapeutic Tai Chi for Older Adults and N’Balance for fall prevention.

COAW’s regional coordinator, Jo Stinchcomb, works from the SECAHEC office.  She provides technical assistance and support to class leaders and partnering agencies throughout southeastern Colorado.  All of the individuals who teach COAW’s courses receive extensive training in course content and instructional skills.  For information on attending a class becoming a partner organization, training as a leader, and hosting a community class go to www.coaw.org or contact:
